Service Descriptions

Medicare Certified Home Health Care

Based on a plan of care established by a physician, a variety of medical-related services are available to individuals who meet the homebound criteria. Services include skilled nursing, physical therapy, occupational therapy, speech-language pathology services, wound care, medical social services, and other assistance provided in the individual’s home. Services are available to residents in Jewish Senior Life communities and are also open to the community. Skilled home health services are billed to and paid for by Medicare and private insurance.

Services include supports to help an individual age in place or to assist people with disabilities. These include personal care, housekeeping, laundry, meal preparation, medication reminders, respite and shopping.

Services include a needs assessment, development of a self-sufficiency plan, information and referrals and help with navigating public benefits. Financial assistance with basic living needs for those who meet eligibility criteria.
